I accept the following insurance payments:

• Deseret Mutual Benefit Advisers (DMBA)

• EMI Health

• PEHP

• Regence (i.e. Blue Cross/Blue Shield plans in Idaho, Oregon, Utah, & Washington)

• SelectHealth

• University of Utah Health Plans (previously known as HMHI)

And the following non-insurance payments:

• Cash Pay Rates

  • 30-minute session:  $75

  • 45-minute session: $115

  • 60-minute session:  $150​

• Superbills options

  • ​I am happy to provide what is called a "Superbill" of each session you pay the  above cash-pay rates so you can submit that document to your insurance company and request reimbursement from your insurance company

• Ecclesiastical Payment/"Bishop's Pay" options 

  • I am happy to consult with an Ecclesiastical or Bishop who will be funding your sessions​
